OCHF Grants
OCHF is proud to provide our community with small grants to support health related needs. The foundation accepts grant requests from non-profits that serve our local community each quarter (March 15, June 15, September 15, and December 15). Specifically, the foundation is interested in requests that address physical health issues such as obesity, cancer, or chronic diseases; and mental health issues such as addiction, depression, or suicide.
Internal Department Grants

Please submit an internal grant application to your supervisor or manager making your request and dollar amount.

Manager/Supervisor will present to Executive Leadership Team for review.

Upon approval, Executive Leadership Team will submit to Foundation for grant committee review.

The Foundation Grant Committee will review each quarter (March, June, September, and December) and recommend to full foundation board for approval.

Approval or Rejection will be communicated to applicant.
External Community Grants

Please submit grant application to the foundation by March 15, June 15, September 15, or December 15th. One application is allowed each year.

The foundation director will review and present to the foundation grants committee.

The Foundation Grant Committee will review each quarter and recommend to full foundation board for approval.

Approval or Rejection will be communicated to applicant immediately after the full board meeting.
